Back to Health Pathway

This project will support patients back to health, from “Living Well” in their communities, to “Waiting Well” as they are on lists for diagnosis or treatment, to “Getting Well” whilst they are in the care of the health service to “Recovering Well” back in the community.

We are working with statutory partners such as NWL Integrated Care System together with voluntary sector organisations like Third Sector Together, Helpforce around volunteering in priority neighbourhoods in Brent to:

  • Improve health and wellbeing.
  • Reduce pressure on NHS services.
  • Create valuable and meaningful roles for volunteers.
  • Strengthen the existing community volunteering capacity.

Volunteering Opportunities

Back to Health volunteers will:

  • Offer comfort calls to patients waiting for appointments, procedures or treatment.
  • Help patients to prepare for their appointments, e.g. transport schemes.
  • Support patients to access their telephone or video appointments.
  • Accompany patients to appointments, where needed.
  • Support patients whilst in hospital thorough existing volunteering roles.
  • Signpost patients to community support.
